Medical school will not change and hasn’t changed so any hope new drs will be more compassionate or interested in the person, not gonna happen. Everything he wrote about in 1979 is still happening and worse today.

A little girl down the street wound up living there with about 8 other people, including non-relatives, because her family business was shut down. I could go on.
Was it being starved and tortured in a Soviet gulag or something? No. But asking that my tax dollars (that would have been used on ballet lessons or a week of camp or something) not be used to create a virus then destroy civil liberties as some sort of fucked up Hegelian response isn't a high expectation of the United States government beaurocracy.

My massive eye opener to human medial incompetence, came 30 yrs ago when my mother's cardiovascular issues had me tagging along to appointments. At one point with her in the emergency room, I had to remind the head of the department that veterinarians have more compassion for pets than "you people" have for human beings (in front of everyone there - ya, I was ticked!). He didn't realize a competent medical professional was listening to all the gobbledygook he was feeding my mother. The doctor guy was going to "try something"... that could cause her to go into cardiac arrest. But "that's ok, at least you're in the emergency room." So, I promptly took over telling him what he was going to do, instead of killing my mother (and getting paid). Happy ending: she survived the trip to the hospital. (The problem was that a test they ran reported out spurious results - and she was incorrectly treated based on that.)
Jim, your conclusion that medicine may devolve into small clinical settings with less sophisticated and more affordable CARE, and where doctors won't be driving German cars, earning a middle class living. Exactly what I said many years ago, i.e., to practice more like veterinary clinics...and where clinics must compete for customers. The key to controlling costs is to require all medical providers to publish a price list and to get the patient's signature on the exact price to be charged, in advance of treatment. No surprise billing, only shopping for cost vs quality.
Obviously there can be no market in emergencies, so emergency prices must be limited by law to whatever Medicare pays. Again, no surprise billing.

I was a CAR T patient at MD Anderson in Houston during this period. I remember how spooky it was to see this huge, normally teeming facility, reduced to a ghost town.
I was told more than once that they were operating a reduced staffing of around 25%.
That noncovid patients like myself were treated in the main campus, "covid positive" patients went to the Mays Clinic across the street.
This reduction of anything not covid, was a widespread policy advocated by our federal government and the medical community in general.
But over time, the media would tell us that the increase in deaths not related to covid, were because the fearful public had simply chosen not to get medical care.
The downstaffing was apparent here, across the entire Texas Medical Center. The medical community did nothing to allay those fears, and encourage the public to get any acute or prevetative care.
TikTok nurses danced in hallways while claiming their own victim creds, as our elderly loved ones died in isolation.
